Concussion Awarness Resources

Per the Concussion Policy and Concussion Code of Conduct, I recognize that participants under the age of 26 years old must review the applicable Concussion Awareness resources at least once a year:
  • Ages 10 and under
  • Ages 11-14
  • Ages 15+

    • Parents/Guardians of participants under 18 years old, and coaches and trainers who are interacting with athletes under the age of 26 years old, must also review the Concussion Awareness resources.

    By signing this Registration Form, I certify for myself and/or on behalf of my participant who is under 18 years old, that the Concussion Awareness resources have been reviewed.

    CONCUSSION CODE OF CONDUCT

    Per the Concussion Policy and Concussion Code of Conduct, I recognize that participants under the age of 26 years and Parents/Guardians of participants under 18 years old, old must review and sign the Concussion Code of Conduct located here:  Concussion Code of Conduct - Participant  

    By completing this Registration Form by typing/printing my name, the date below and clicking/checking the “I Agree” icon I certify for myself and/or on behalf of my participant who is under 18 years old, that we have fully reviewed and commit to this Concussion Code of Conduct.

    CONSENT FOR USE OF PERSONAL INFORMATION AND PHOTO RELEASE

    I, the undersigned, authorize Guelph Minor Softball Association (GMSA)  to collect and use personal information about the Registrant for the purpose of receiving communications and the purposes described in the GMSA allprivacy policy.

    Furthermore, I grant permission to GMSA  to photograph and/or record the Registrant’s image and/or voice on still or motion picture film and/or audio tape, and to use this material to promote the sport and/or the GMSA through the media of newsletters, websites, television, film, radio, print and/or display form. I understand that I waive any claim to remuneration for use of audio/visual materials used for these purposes. 

    I understand that I may withdraw such consent at any time by contacting either Organization’s Executive Team. The Executive  will advise the implications of such withdrawal. 

    ACCEPTANCE OF TERMS AND CONDITIONS

    In consideration of the acceptance of the Registrant’s participation with GMSA, I agree that the Registrant will: Abide by the policies, rules and regulations of GMSA; and Accept sole responsibility for the Registrant’s personal possessions and athletic equipment.

    RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT

    The Participant listed above and/or the Participant’s parent/guardian (if applicable, when the Participant is younger than 18 years old) must must accept the terms of this Agreement before participation. When the Participant is a minor, the Participant’s parent/guardian acknowledges and agrees to the terms on behalf of the Participant, and references in this document to the Participant agreeing to or acknowledging a risk or term is understood to be referring to the Participant’s parent/guardian agreeing to or acknowledging the risk or term on behalf of the Participant.

    Waiver of Liability 

    By typing/printing your name, the date below and clicking/checking the “I Agree” icon, you agree that you are to be bound by all that is contained in this Agreement
